Hope for People with Chronic Illness

More than 166.6 million people in the US live with at least one chronic health condition. The experience of living with a chronic illness is common, yet still misunderstood.

 

Chronically Informed serves chronically ill patients and their caregivers by showcasing videos that share stories of success and practical techniques for navigating the medical system. These videos create hope for patients by helping them understand that others have found ways to navigate the medical system and inspire patients to use those pathways for themselves.

Chronically Informed creates hope for chronically ill patients, fosters patient-provider partnerships, expands healthcare access, and empowers patients through shared experiences navigating the medical system.

Dr. Jessie Shay is the Founder and Executive Director of Chronically Informed, which is a project of the Chronic Care Collaborative, and operates as a 501(c)(3). It is fiscally sponsored by the Fund for a Healthier Colorado/The Archer Group.
